2. Prepare Gauze Pad, Bandage and Lancet

Before proceeding you need to have fasted for 8 hours prior to collecting your blood so it is best to use this test in the morning after a good night's sleep as long as 8 hours have passed since eating.

3. Prepare Glucose Test

4. Prepare Test Finger

Make sure the finger is clean and warm by rubbing your finger with the enclosed alcohol prep pad.

5. Use Finger-Nick Lancet

Remove cover and put red edge flat against finger. Press quickly.

6. Wipe Away First Drop of Blood

Using gauze pad provided.

7&8. Collect Blood Sample by Filling Test Area with Blood Sample

 

     

 

Keep standing as it makes blood collection easier.

9.  Allow 3 Minutes for Test Results

10.  Read Instant Glucose Result

Read instant Glucose results by matching the green shade of the test area to the same green shade of the reference card. Find the best match. Turn over the card to obtain your glucose value.

If your Glucose results are below 75mg/dL, you are done.

11.  Interpretation Guide

If your results are below 75mg/dL	Congratulations! You are at low risk for developing diabetes.
If your results are between 75-100 mg/dL	You are at medium risk for developing diabetes. You should eat a healthy diet and exercise and test again in 3 months. If you want more details results, send your rapid response device to our certified laboratory.
If your results are between 100-150mg/dL	You are at high risk for developing diabetes and should send your rapid response device to our certified laboratory for further testing.

12.  Fill Out Laboratory Authorization Form

1. Complete the Laboratory Authorization Form accurately.

2. Note the date and time of blood collection and record your Glucose Results value where indicated.

3. Select Payment Options.

4. Write down your specific number on the bottom of this form.

13.  Did You Remember To?

1. Remove one bar code label from the Lab Authorization Form and apply to card as shown in step 3.

2. Fill out Laboratory Authorization Form.

3. Provide check or payment information for $10 to Healthy Life Laboratories.

4. Sign Consent.

5. Enclose and seal the test card in foil sample return bag.

14.  Laboratory Confirmation

1. Send authorization form and foil pouch with enclosed top tab using the enclosed business reply envelope.

2. Enclose $10 check or payment information to Healthy Life Laboratories.

3. All of the blood collection materials are single-use and disposable. Discard items according to your local waste disposal policies and regulations.
